The manuscript comes from Klášter Teplá (Stift Tepl) in Bohemia: Milo Nentwich, “Zur Geschichte der Tepler Stiftsbibliothek.” Beiträge zur Geschichte des Stiftes Tepl, vol. 1 (Marienbad 1917), pt. 2, p. 89: s. XIV; Explicit: “Explicit decretum compilatum gratiano monacho monasterii sancti felicis. De Bononia.”

Note that Kristeller-Krämer seem to indicate that this collection was still in Teplá. In fact, the historical collections of Teplá Abbey were still kept in Teplá even after the monastery was dissolved in 1950, but that it sold its medieval manuscripts to the Czech National Library between 2006 and 2008, presumably including a Gratian copy owned by the library in 1996 according to the FABIAN handbook.
