The manuscript Bologna, Biblioteca del Convento di San Paolo in Monte (dell'Osservanza) Y 6.6 is listed as a copy of Gratian by Mirabile (here). However, also according to Mirabile (here), Stevanin/Zanardi mention Y 6.6 as an incunabulum in the San Paolo library, and still according to Mirabile (here) Tursi traced the extant medieval manuscripts of the convent, none of which still in situ. Apparently, therefore, the reference is misguided.
