Brescia, Museo Diocesano di Arte Sacra, Codici miniati ms. 3 is a twelfth-century copy of Bonizo's Liber (fol. 1r-131r) and his Libellus de sacramentis (fol. 131r-132v). The codex was discovered in the mid-eigteenth century in the chapter library of Brescia cathedral and studied, among others, by the Ballerini brothers.

The manuscript, which had no shelfmark in the chapter library, today is kept in the Museo Diocesano di Arte Sacra with the new shelfmark Codici miniati ms. 3. Hence the older literature including Perels, Schadt and Kéry cites it as "Brescia, Archivio (e Biblioteca) Capitolare, sine numero".
