Città del Vaticano, Biblioteca Apostolica Vaticana, Vat. lat. 3787 (written shortly after the year 1000) is the earliest and most important copy of the Collectio Avellana (V in Günther's edition). The only other medieval mansucript (Vat. lat. 4961) is a direct copy of it, and all modern manuscripts are copies of either Vat. lat. 3787 or Vat. lat. 4961.
